What do I need to do?

1 - Please click HSA Request on the bottom right of the screen to display the form

2 - Populate your, Name, Email Address, Property Address and Description

3 - The customer services team will send your HSA via Docusign

4 - You Review and Sign your HSA

5 - The Customer Services team wil send your Online Glass Activation Email so you can manage your account online
